If you have Brown & Toland insurance coverage (check the back of your plastic insurance card), we will be delighted to see you. Please request a referral for examination from your primary care physician.

In addition to the routine referral, Brown & Toland requires special permission to do certain procedures such as injections of Sculptra for Facial lipoatrophy. Please make an appointment to see us, with a referral from your physician, for a routine evaluation. We will submit the necessary paperwork to Brown & Toland to secure authorization for treatment of facial lipoatrophy.

For patients with all types of insurance, please bring your insurance card and government issued ID to your appointment.
